Tired of Harassment from Collectors for a Debt Paid

Case Description
I had a loan through TD-Canada Trust about 15-20 years ago co-signed by my mother now in her mid 70's which was paid off in full in 2002 ( I have all the paperwork to prove it).It seems about every 3-4 years it goes back into collections and I get this annoying letter saying I still owe $400.00.( It keeps going up everytime also).I have finally had enough and want to get a laywer to get rid of this once and for all.I want to sue TD and this collection agency for harrassment and affecting our credit scores to me and my retired mom who doesn't need this aggrevation.
